A contractor struck and damaged an 11kV cable whilst directional drilling. One phase of the cable was damaged and as such only one of the DDO fuses operated.

An approximate 4m x 1350mm pipe, was being loaded onto a truck using a Merlo Telehandler. However, the pipe slipped from the Telehandler's jaws and rolled off the opposite side of the truck, resting on the ground beside the vehicle.

Contractor was welding a flange onto a steam gland pipe when a powdery white substance inside a pipe has ignited. Substance is a combustible material applied to pipes prior to shipping.
