Access to LV while Kiosk Energised
Date of Incident: 27 June 2018
Description:
The BoP operator proceeded to unlock the LV 690v double doors of a TX Kiosk and then stood back, took a photo of the blushing, closed the double doors and then locked the LV door.
Signage in the LV area states that the LV doors must not be opened unless transformer has been isolated and earthed. This was not followed.
At no time did the BoP operator encroach the MAD of 150mm.
