Release of water under pressure

Description:
Defect work to service the Fire System Pressure Relief Valve (PRV) and Pressure Regulating Valves (sometimes also referred to as PRV) which required removing the valves from service. WA issued but the appropriate RASM's were not in place for the Pressure Regulating Valve resulting in a sudden release of pressurised water (20 bar).
