Truck Park Brake Valve Failure

Description:
Truck rolled 50 metres down hill after chocks were removed. A mechanical assessment found that the park brake failed and did not exhaust the air from the park brake circuit. The failure left the truck in a free rolling state, the vehicle had been left in an engaged state (idling).
