Mighty Evac Failure

Description:
During confined space rescue training we were winching a person up out of a Piezometer Hut using the Mighty Evac. During this process, the Mighty Evac failed to hold in the winch/rescue mode and the person fell approximately 500mm before we got the winch under control.
No injury occurred to the person.
