Pre-use Vehicle Checks

Date of Incident: 13 June 2018
Description:

While travelling, a ute gull wing toolbox catch became loose and resulted in the toolbox hatch accidentally opening.

A prop valve and two screwdrivers fell out of the toolbox while driving around a bend. The driver realised what had happened immediately and pulled over in a layby 30m on. No vehicles were behind the ute at the time.

A similar event occurred at Mt Mercer Wind Farm on 20 June 2018 involving a contractor flat deck ute. A battery-powered high torque unit wasn't tied down and as the vehicle rounded a bend, the tool in its case fell off. The tool was damaged beyond repair.
