Radio Drop From Harness Pouch

Description:
A radio disengaged from the latch lock on a Python radio holster and fell out onto a turbine platform 20 meters below causing the radio to break. The radio holster was securely fixed to the harness at the time. Examination of the holster has identified that the retaining adjustment button can be pulled through if a stop knot has not been tied at the end.
