Description:
During a planned visit to a Siemens wind turbine at West Wind, a Meridian technician was performing the pre-use inspection on a Skyman tower hoist/lift type TH250A.
During the inspection, the safety brake was identified as slipping and, as a result, failed its pre-use check. The service hoist was then locked and tagged out.
