Release of Fall Arrest Lanyard

Description:

During working at height training, a Wind Technician was undertaking a ladder rescue approx. 6-7m up. Their dorsal double action hook managed to unlock itself from the dorsal attachment point on his harness. As his back made contact with the towed, the hook became disengaged from his harness. This resulted in the technician having no protection in the event of a fall.

The technician had 4 points of contact to the ladder at the time.
